First, Let's Review Required Documents

Please review the list below and ensure you have everything (filled out, signed, saved) and ready to upload, before moving on.

The first 3 documents were provided to you when you applied for your license.

  • Resident State License Number

  • NPN (National Producer Number)

  • Copy of Resident State License

Step 1: Obtain E&O Certificate

What is E&O?

An Errors and Omissions (E&O) certificate is a required certificate that provides professional liability coverage for insurance agents in case of errors in services. To learn more, please visit the link below.

  • If you have your E&O Certificate, please have it ready to upload.

  • If you do NOT have your E&O Certificate, please visit the link below to obtain.

Step 2: Obtain EFT Form

IMPORTANT NOTE:

You DO NOT need to attach verification of your bank account.

You DO NOT need to attach a Voided Check, Deposit Slip or Bank Letter.

You ONLY need to FILL OUT TOP HALF, SIGN, DATE AND UPLOAD the EFT form
